Kindergarten Registration
Your child will need a physical exam, as well as all immunizations being up-to-date or in the process of being completed by the first day of school.
The immunizations required are as follows:
DPT/DT- 4 doses minimum, last being after age 2 yr.
Polio- 4 doses, but 3 doses are enough if at least one was given after the second birthday.
MMR- 2 doses
Hepatitus B- 3 doses
Please note that there is a new State requirement regarding Hepatitis B immunization and MMR immunization. All students enrolling in the Fall 1997 are required to be immunized against Hepatitis B. This immunization is given in a series of three doses.
In order to enter Kindergarten the Hepatitis B immunization must either be completed or in the process of being completed.
A second dose of the MMR immunizaton is required before entering Kindergarten.
Please call or come by the office to pick up a registration folder for your kindergartner. These forms need to be completed and returned to the District Office, along with the birth certificate and immunization records of your child.
The Physician's Report should be completed by your family doctor at the time of the child's physical and returned to the school office.

Here are the steps a parent needs to follow in order to register your child for kindergarten:
1. Child must be 5 on or before December 1 of the year they are entering kindergarten- for the 2008-09 school year, it is December 1, 2008.
2. Documents required include birth certificate - a copy is fine- and current immunization records. You may go to your health provider and ask them to provide you with a complete signed record of immunizations. Students WILL NOT be admitted without current and appropriate immunizations. Your health provider can assist you in identifying what is required.
3. A physical exam is required before school entrance. You will need to provide proof of that exam at the time of registration.
Special note: There is a new immunization for incoming kindergarten students. They are now required to have one dose of VARICELLA ( chicken pox) prior to school entry, or you need to provide written documentation from a doctor or clinic stating that your child has had the chicken pox.
Other required immunizations:
DPT - minimum 4 doses, last dose after age 4
Polio- minimum 3 doses last dose after age 4
MMR- 2 doses, after age 1
Hepatitis B- 3 doses
Personal belief exemptions are allowed, Waivers need to be signed in the school office.
